Although the title of this form is “Marriage Request Form,” this form is actually intended to be used to gather information to initiate anything relating to the Sacrament of Marriage at St. Thomas More. If you are interested in celebrating your wedding here, are seeking a convalidation or are interested in marriage prepration for a wedding at another church, this is the form to complete.
